Please note that this schedule is a little different than the one listed on the waiver form. This schedule gives me time get a check ready from the school before the actually payment is due to the trip company. Please stay current with payments. Thank you!

Every person going on the trip is required to have one of these on file. This form must be notarized and must be accompanied by a copy of a photo id. If you do not have a photo id, please let me know.
